Positional asphyxia occurs when a person’s body position restricts breathing, leading to insufficient oxygen intake and potential death. This form of mechanical asphyxia can affect both adults and infants, though the mechanisms and risk factors differ across age groups.

In adults, positional asphyxia often involves immobilization in postures that compress the chest or diaphragm (e.g., head-down positions), increasing intrathoracic pressure and hindering lung expansion. Hyperflexion of the neck or torso can partially or fully obstruct airways, particularly in unconscious individuals. Predisposing factors include intoxication, organic diseases, or physical restraint during altercations, which impair the ability to self-correct positioning.
Infants and toddlers are vulnerable due to underdeveloped neck muscles and airway structures. Slumped positions in car seats or sling carriers may cause the chin to press against the chest, blocking airflow. Premature and low-birthweight infants face higher risks due to weaker respiratory control and smaller airway diameters.
