A Congregate Living Health Facility (CLHF) is a licensed residential medical care facility that provides inpatient skilled nursing care in a home-like, communal environment. It offers care that is more intense than that provided in skilled nursing facilities, but less intense than acute care hospitals. These facilities typically serve individuals who are medically fragile or complex, including those who are physically disabled, ventilator-dependent but mentally alert, or who have life-threatening illnesses or severe disabilities. CLHFs provide 24-hour skilled nursing, medical supervision, pharmacy, dietary, social, and recreational services.
CLHFs usually have a limited number of beds (often under 18) and focus on subacute care—care for patients transitioning down from general acute care hospitals or those needing ongoing medical attention not feasible at home or in traditional skilled nursing facilities. They are designed to help patients live fulfilling lives in a homelike setting while receiving continuous medical care.
These facilities are licensed by the state’s Department of Public Health. They work with hospitals and insurance providers and cater to patients with complex medical needs such as stroke, traumatic brain injury, severe lung disease, heart failure, and spinal cord injuries.
