Los Angeles County is reporting the death of a 47-year-old Hispanic female.

The coroner’s office has identified the woman as Yvonne Galindo.

Manner of Death: Accident

Cause of Death: Effects of Fentanyl, Cocaine, and Phencyclidine

Formal pronouncement of death was made on Wednesday at 6:03 a.m.

The Department of Medical Examiner publicly disclosed the CoD ruling on March 20, presumably after the results of tox screening had come back positive for the drugs.
