Los Angeles County is reporting the death of a 44-year-old Latino male that occurred at a private residence.
The coroner’s office has identified the man as Jose Vasquez.
Born in L.A. County. Mother’s maiden name is Gonzalez.
Manner of Death: Accident
Cause of Death: Effects of Fentanyl, Cocaine, Ketamine, and Ethanol

Formal pronouncement of death was made on Thursday at 1:54 p.m.
The following three persons were all pronounced dead at the same time:
- John Mendez – own residence
- Simone Ling – own residence
- Jose Vasquez – private residence (not his residence)
Fentanyl-related mass-overdose events, characterized as three or more overdoses occurring close in time and at the same location, have been happening frequently across the country this past year.
The Department of Medical Examiner publicly disclosed the CoD ruling on March 27, after the results of the tox panel had come back from the lab.
