Los Angeles County is reporting the death of a 46-year-old White/Caucasian male that occurred at his residence.

The coroner’s office identified the man as Chad Mataisz.

Manner of Death: Accident

Cause of Death: Acute Methamphetamine and Fentanyl Toxicity

Other Significant Conditions: Chronic Methamphetamine Use, Chronic Fentanyl (Opioid) Use, Recent Discharge From Substance Abuse Rehabilitation
