(L.A.) Yaakov Moskowitz, 39, Died at a Treatment Center

Los Angeles County is reporting the death of a 39-year-old White/Caucasian male that occurred at a treatment center.

The coroner’s office identified the man as Yaakov Moskowitz.

Manner of Death: Accident

Cause of Death: Combined Effects of Buprenorphine, Naloxone, Quetiapine, and Trazodone

Formal pronouncement of death was made on Sunday.

Ruling by the deputy medical examiner was published on February 2, presumably after the results of tox screening had come back positive from the lab.

This is considered an accidental, i.e., unintentional, polydrug overdose.
