Los Angeles County is reporting the death of a 28-year-old Caucasian male that occurred in a hospital.

The coroner’s office has identified the man as Tomas Malhocky.

Manner of Death: Accident

Cause of Death: Combined Effects Of Fentanyl, Cocaine, Methamphetamiine, and Methylenedioxymethamphetamine (MDMA)

Other Significant Condition: Hypertrophic Heart Disease
