The coroner’s office has identified the individual as Thomas Oshaughnessy.

Cause of Death A: Ischemic Heart Disease

Cause of Death B: Atherosclerotic and Hypertensive Cardiovascular Disease

Other Significant Conditions: Medical History of Congestive Heart Failure, Atrial Fibrillation,
Hypertension, Obesity, and Pressure Ulcers
