(L.A.) Theodore Scalise, 74, Died in May

Los Angeles County is reporting the death of a 74-year-old White/Caucasian male.

The coroner’s office has identified the man as Theodore Scalise.

Manner of Death: Accident

Cause of Death: Combined Effects of Fentanyl, Alprazolam and Ethanol

Other Significant Conditions: Medical History of Tobacco Smoking; Prior Drug User of Speed and Heroin; Unspecified Psychiatric Treatment

RIP THEODORE SCALISE (May 26, 1948 – May 10, 2023)

Formal pronouncement of death was on Wednesday at 7:23 a.m.

Ruling by the deputy medical examiner was published on January 3, presumably after the results of tox screening came back positive for the drugs.

The decedent appears to have been the drummer TS Waters of the L.A. pop rock band Made in Japan.

Katrina Hale said she received a call from her stepson after she was told he had died.

The Los Angeles County coroner’s office tells NBC4 they made a rare mistake and misidentified a body discovered in downtown Los Angeles.

If Hale had not brought it to the coroner’s attention, the deceased would have been cremated and the wrong family informed of his death.
