Los Angeles County is reporting the death of a 58-year-old White/Caucasian male that occurred at a healthcare service.

The coroner’s office identified the man as Sean Blair.

Manner of Death: Accident

Cause of Death: Combined Effects of Citalopram and Fentanyl

Other Significant Conditions: Congestive Heart Failure, Bipolar Disorder, Chronic Ulcer of the Left Foot, Atrial Flutter, Diabetes Mellitus, Depression, and a History of Suicidal Ideation

Formal pronouncement of death was made on Saturday at 2:29 p.m.

Ruling by the deputy medical examiner was published in November, presumably after the results of tox screening had come back positive for the drugs.
