Although aortic dissection is uncommon in younger, non-obese individuals, it can present atypically with upper abdominal pain, nausea, and vomiting, mimicking a benign gastrointestinal illness and delaying recognition until catastrophic rupture occurs. In fact, a significant proportion of aortic dissections (approximately 20–30%) lack classic chest pain. Instead, patients may report upper abdominal discomfort, nausea, and vomiting, reflecting visceral involvement or compromised mesenteric perfusion. These symptoms can precede catastrophic rupture and hemopericardium by several hours.
This atypical presentation is thought to result from visceral ischemia, autonomic nervous system activation, or extension of the dissection into the descending aorta and its branch vessels. Gastrointestinal-type symptoms may appear well before hemodynamic collapse, rupture, or cardiac tamponade, which contributes to frequent misdiagnosis as a primary gastrointestinal illness.
In this case, there was effectively no reliable point at which a reasonable clinician could have anticipated the catastrophic outcome or meaningfully altered it. Even if the dissection had been recognized promptly in the emergency department, definitive repair would have required emergent open-heart surgery, carrying a high risk of mortality. Once rupture with hemopericardium occurs, survival is unlikely despite optimal medical and surgical intervention.
