Los Angeles County is reporting the death of a 36-year-old Black male that occurred at an adult care facility.
The coroner’s office identified the man as Lonnie Alexander.
Manner of Death: Accident
Cause of Death: Acute Intoxication by the Combined Effects of Chlorpheniramine, Methorphan, and Olanzapine
Other Significant Conditions: Hypertensive and Atherosclerotic Cardiovascular Disease; Diabetes Mellitus

Olanzapine is a prescription antipsychotic medication, often sold under the brand name Zyprexa.
Dextromethorphan is an OTC cough suppressant found in products such as Robitussin Cough, Vicks DayQuil Cough, or Mucinex DM.
Chlorpheniramine is an OTC antihistamine commonly used for allergies and colds, found in brands like Chlor-Trimeton, Aller-Chlor, and various combination products, including Children’s NyQuil Cold & Cough.
Where a person prescribed olanzapine dies after also taking OTC chlorpheniramine and dextromethorphan in a care facility, it is possible that a medication error, lack of awareness of the dangerous interactions, or insufficient monitoring contributed to the fatal outcome.
