Cause of Death A: Ischemic Heart Disease

Cause of Death B: Atherosclerotic and Hypertensive Cardiovascular Disease

Other Significant Conditions: Medical History of Congestive Heart Failure, High Blood Pressure, Hypertension and Chronic Obstructive Pulmonary Disease
