Cause of Death A: Congestive Heart Failure

Cause of Death B: Hypertension

Other Significant Conditions: Diabetes Mellitus Type 2, End-Stage Renal Disease, Dementia, Schizophrenia, Dysphagia, Esophageal Stricture, Gastroesophageal Reflux Disease
