“Acute on chronic” is a medical term that describes an acute (sudden) worsening or exacerbation of a chronic (long-term) condition.
“Refractory” means resistant to treatment or cure. In medical contexts, it describes a condition that does not respond well to typical therapies.
In septic shock, “refractory” indicates persistent hypotension and inadequate tissue perfusion despite standard resuscitation measures.
