Los Angeles County is reporting the death of a 31-year-old White/Caucasian male that occurred at his residence.

The coroner’s office has identified the man as Samuel Chavez.

Cause of Death: Effects of Fentanyl and Ketamine

Formal pronouncement of death was made on Wednesday at 8:35 a.m.

This is considered an accidental, i.e., unintentional, drug overdose.
