Los Angeles County is reporting the death of a 60-year-old Black female that occurred in a hospital.

The coroner’s office has identified the woman as Jeannette Chism.

Manner of Death: Accident

Cause of Death: Cocaine Toxicity

Formal pronouncement of death was made on Saturday at 4:42 a.m.

The Department of Medical Examiner publicly disclosed the CoD ruling on June 15, presumably after the results of tox screening had come back positive for the drug.
