Los Angeles County is reporting the death of a 23-year-old White/Caucasian male that occurred in a group home.

The coroner’s office has identified the man as Jack Haluska Barnes.

Manner of Death: Accident

Cause of Death: Effects of Fentanyl and Methamphetamine

Formal pronouncement of death was made on Tuesday at 11:38 a.m.

The Department of Medical Examiner publicly disclosed the CoD ruling on March 31, presumably after the results of tox screening had come back positive for the drugs.
