(L.A.) Jack Anicito, 24, Died at a Rehab Facility

Los Angeles County is reporting the death of a 24-year-old male that occurred at a rehab facility.

The coroner’s office identified the young man as Jack Anicito and listed his race/ethnicity as Hispanic [sic].

Manner of Death: Accident

Cause of Death: Fentanyl and Ketamine Toxicity
