Los Angeles County is reporting the death of a 60-year-old Black male that occurred in a hospital.

The coroner’s office has identified the man as Dale Toliver.

Manner of Death: Accident

Cause of Death A: Cardiopulmonary Arrest

Cause of Death B: Exsanguination

Cause of Death C: Disruption of Arteriovenous Fistula

Cause of Death D: End-Stage Renal Disease Requiring Dialysis

Other Significant Conditions: Cardiomegaly, Atherosclerotic Cardiovascular Disease, Hypertension, Diastolic Heart Failure

Formal pronouncement of death was made on Saturday at 8:10 a.m.

Ruling by the deputy medical examiner was published on April 23.
