Los Angeles County is reporting the death of a 40-year-old White/Caucasian female.
The coroner’s office has identified the woman as April Yost.

Cause of Death A: Multiorgan Failure
Cause of Death B: Acute Respiratory Distress Syndrome
Cause of Death C: Toxic-Metabolic Encephalopathy
Cause of Death D: Probable Serotonin Syndrome

Formal pronouncement of death was made on Sunday.
Ruling by the deputy medical examiner was published on August 19.
This was classified as an accidental and not natural death because it was the unintentional consequences of a drug interaction that triggered a rapid, uncontrollable, and fatal chain of organ failure throughout the body.
Serotonin Syndrome is a drug reaction caused by an excessive accumulation of serotonin in the body. It’s usually caused by taking a combination of drugs that increase serotonin levels (e.g., SSRIs antidepressants, SNRIs, certain migraine medications, illicit drugs like MDMA/Ecstasy, or even a single drug in high doses).
The medical examiner uses “probable” because a definitive diagnosis of Serotonin Syndrome can be challenging. It is often a clinical diagnosis based on symptoms and medication history rather than a single definitive test. The evidence strongly pointed to it, but it might not have been 100% confirmed ante-mortem (before death).
“Encephalopathy” means a disorder or disease of the brain. “Toxic-Metabolic” means it was caused by a toxin (in this case, the drug toxicity from the serotonin syndrome) and a resulting metabolic imbalance (e.g., extremely high fever, dehydration, acid-base imbalance, or organ failure affecting the brain).
The body-wide crisis from Serotonin Syndrome (high fever, agitation, organ stress) creates a toxic internal environment. This disrupts the brain’s function, leading to severe confusion, delirium, seizures, or coma.
