Los Angeles County is reporting the death of a 68-year-old Black male that occurred at a long-term care facility.

The coroner’s office identified the man as Andrew Lewis.

Manner of Death: Accident

Cause of Death: Combined Fentanyl and Methamphetamine Intoxication

Other Significant Conditions: Hypertensive Heart Disease, Type 2 Diabetes Mellitus, Chronic Kidney Disease, and Paranoid Schizophrenia
