Los Angeles County is reporting the death of a 36-year-old Hispanic male that occurred at a residence.

The coroner’s office has identified the man as Adolfo Madrigal. Born in L.A. County.

Manner of Death: Accident

Cause of Death: Fentanyl and Cocaine Toxicity

Formal pronouncement of death was made on Saturday at 11:24 a.m.

Ruling by the deputy medical examiner was published on August 14, presumably after the results of tox screening came back positive for the drugs.

The DME had published this record on June 24 with the name Adolfo Madrigal, Jr. On June 27, they removed the “Jr.”
