Los Angeles County is reporting the death of an 18-year-old Hispanic female that occurred in a drug / alcohol rehabilitation facility.

The coroner’s office has identified the woman as Destiny Flores.

Manner of Death: Accident

Cause of Death: Fentanyl Toxicity

Formal pronouncement of death was made on Tuesday at 6:03 a.m.

The Department of Medical Examiner publicly disclosed the CoD ruling in late March, presumably after the results of tox screening had come back positive for the drug.
