Los Angeles County is reporting the death of a 76-year-old White/Caucasian female that occurred in a hospital.

The coroner’s office has identified the woman as Phyllis Hammack.

Manner of Death: Accident

Cause of Death A: Sequelae of Blunt-Force Injuries

Cause of Death B: Fall

Other Significant Conditions: History of Hypertension, Coronary Artery Disease, Cirrhosis, Chronic Obstructive Pulmonary Disease, and Heart Failure
