(L.A.) Rafael Pacheco-Carrillo, 31, Died From Substance Abuse

Los Angeles County is reporting the death of a 31-year-old Latino male that occurred in a hospital.

The coroner’s office has identified the man as Rafael Pacheco-Carrillo.

Cause of Death: Effects of Fentanyl and Ethanol

RIP RAFAEL PACHECO-CARRILLO (September 29, 1992 – September 13, 2024)

Formal pronouncement of death was made on Friday at 3:03 a.m.

Ruling by the deputy medical examiner was published in mid-November, presumably after the results of tox screening had come back from the lab.

This is considered an accidental, i.e., unintentional, drug overdose.
