Los Angeles County is reporting the death of a 45-year-old White/Caucasian male that occurred at a hotel.

The coroner’s office identified the man as Philip Kwait.

Formal pronouncement of death was made on Sunday at 5:40 p.m.

The government investigator had initially misspelled the decedent’s forename as Phillip, later making the correction.

* In mid-November, the Department of Medical Examiner changed the manner of death from suicide to accident, and also added the line about ketamine toxicity.
