The coroner’s office has identified the man as Keith Chalk.

Manner of Death: Accident

Cause of Death: Sequelae of Blunt Trauma

Other Significant Conditions: Hypertensive Cardiovascular Disease, History of Chronic Polysubstance Abuse

Formal pronouncement of death was made on Thursday at 5:08 p.m.

The Department of Medical Examiner publicly disclosed the CoD ruling in early June.
