(L.A.) Susan Chacon, 60, Died at a Private Residence

Los Angeles County is reporting the death of a 60-year-old Hispanic female that occurred at a private residence.
The coroner’s office identified the woman as Susan Chacon.
Born in L.A. County. Mother’s maiden name is Bradford.
Manner of Death: Accident
Cause of Death: Combined Effects of Ethanol, Duloxetine, Quetiapine, and Hydrocodone
Other Significant Conditions: Medical History of Chronic Obstructive Pulmonary Disease, Depression, Anxiety, Schizophrenia, Emphysema, Polysubstance Drug Abuse, and Tobacco Smoking

Formal pronouncement of death was made on Friday at 10:30 p.m.
The taxpayer-paid government investigator had initially posted the decedent’s name as Chacon, then later inexplicaly changed it to Cahacon, before again changing it back to Chacon.
Ruling by the deputy medical examiner was published on July 7, presumably after the results of tox screening had come back positive for the drugs.
Duloxetine is antidepressant medication used also for treatment of anxiety and chronic pain.
The decedent had a history of pill abuse (Xanax, Vicodin, Clonazepam, and Gabapentin).
