Los Angeles County is reporting the death of a 50-year-old White/Caucasian male that occurred at a residence.

The coroner’s office has identified the man as Samuel Millens.

Manner of Death: Accident

Cause of Death: Fentanyl and Cocaine Effects

Formal pronouncement of death was made on Sunday at 6:59 p.m.

Ruling by the deputy medical examiner was published on August 31, presumably after the results of tox screening came back positive for the drugs.
