Los Angeles County is reporting the death of a 90-year-old Caucasian female that occurred in an assisted living facility.

The coroner’s office has identified the woman as Helen Phillips.

Manner of Death: Accident

Cause of Death A: Consequences of Intracranial Hemorrhage

Cause of Death B: Blunt Force Trauma to the Head

Other Significant Conditions: Alzheimer’s Disease, Arteriosclerotic Cardiovascular Disease
